This checklist should be used to record every work-related death and every work-related injury or illness that involves loss of consciousness, restricted work activity or job transfer, days away from work, or medical treatment beyond first aid. The OSHA 300 – Log of Work-Related Injuries & Illnesses Checklist will allow you to identify and establish any persons involved with the event, and describe and classify the event.
